By registering for Joga Soccer Academy’s youth soccer camp, I, the undersigned, acknowledge and agree to the following terms:

1. Acknowledgement of Risks
   I understand that participating in a soccer camp involves inherent risks of injury, including but not limited to physical injury, sprains, strains, fractures, and other possible accidents. I acknowledge that Joga Soccer Academy will make every effort to ensure the safety of all participants, but such risks cannot be entirely eliminated.

2. Voluntary Participation
   I acknowledge that participation in Joga Soccer Academy’s youth soccer camp is voluntary, and my child has my permission to participate. I affirm that my child is in good physical health and capable of engaging in physical activity.

3. Assumption of Risk
   I, on behalf of my child, voluntarily assume all risks associated with participating in Joga Soccer Academy’s youth soccer camp, including risks of injury that may arise during camp activities, travel to and from the event, and the use of any facilities, equipment, or vehicles provided.

4. Release of Liability
   I hereby release and hold harmless Joga Soccer Academy, its officers, employees, agents, coaches, volunteers, and any other parties involved in the camp from any and all liability, claims, demands, or causes of action of any kind, arising out of or in connection with my child's participation in the soccer camp. This release applies to all claims for personal injury, property damage, or wrongful death arising from or in any way related to participation in the camp.

5. Medical Treatment Authorization
   In the event of an emergency, I authorize Joga Soccer Academy, its agents, or employees to seek emergency medical treatment for my child as deemed necessary. I also agree to assume full responsibility for any costs associated with medical treatment or emergency services that may be required during the camp.

6. Photos and Media
   I consent to the use of my child’s image, likeness, and/or voice in any promotional material, media, website, or social media posts related to Joga Soccer Academy’s activities, without compensation.

7. Indemnification
   I agree to indemnify and hold harmless Joga Soccer Academy from any and all claims, liabilities, and expenses, including attorney’s fees, arising out of my child's participation in the camp, including any actions caused by my child’s own conduct.

8. Severability
   If any portion of this waiver is deemed invalid by a court, the remainder shall remain in full force and effect.

9. Governing Law
   This Release and Waiver shall be governed by the laws of the state/province in which Joga Soccer Academy is based.

Acknowledgment of Agreement
By checking the box above, I acknowledge that I have read, understood, and voluntarily accept the terms of this Release of Liability and Waiver.
